Resignation Information For Flight Attendants

United Airlines Resignation Process - From 10/01/07 Onboard Updates: The Service Center will implement a new process Oct. 1 by which F/As initiate retirement or resignation. These processes previously required the F/A work through their assigned supervisor or via a phone call to the Service Center. The new process allows a F/A to initiate retirement (and resignation including “early out” pass travel) via SkyNet. From the Onboard Service home page, click on the F/A Service Center drop down/Service Center Home/Retirement and Resignation. The Web page also provides policy information and helpful links related to these processes.

Pass Travel Information: Resignation Special Pass Travel Benefit (aka: 20-Year Passes)

Vacation Pay Information - Direct Excerpt from 18.K. of the Contract: F/As who leave the Company, regardless of their length of service with the Company, shall be paid for all accrued but unused vacation credit for the preceding calendar year regardless of the reason for leaving the Company. In addition, F/As who have a full year or more of service with the Company at the time of leaving will receive all accrued vacation credit in the current year up to the end of the month preceding the separation, except when less than ten (10) calendar days notice is given of intent to resign or the F/A is dismissed by the Company. F/As who are furloughed in a reduction in forces will be granted vacation pay for all unused vacation time accrued to the end of the month preceding their furlough.

 California State Disability Insurance (CA SDI) - Short Term Disability

California State Disability Insurance (SDI) is a partial wage-replacement insurance plan for California workers (appears as CA SDI deduction on your paycheck stub). The SDI program is State-mandated, and funded through employee payroll deductions. SDI provides affordable, short-term benefits to eligible workers who suffer a loss of wages when they are unable to work due to a NON WORK-RELATED illness or injury, or a medically disabling condition from pregnancy or childbirth.

Legal Name Change Process (submitted by LAX F/A Miyuki Kanno Boluyt)


1.  Social Security -  Visit your local Social Security office in person to change your name first. Bring your current Social Security Card, your marriage license/court order, and your current DMV license. A letter will be issued by the SS office and is your temporary SS Card until the new card arrives.

2.  DMV - Bring your marriage license/court order, current DMV license, the letter provided by the Social Security office. DMV does not honor your appointment since your name on the SS Card does not match your name on DMV license, so be prepare to stand in line. (You can change your name on the Vehicle Registration and the Voter's registration at the same time, so bring your Vehicle title and get it done.) * Before notifying UAL of your legal name change, you must change the name on your passport.

3.  Passport - You're required to apply for a new passport unless your current one is issued within one year. Make an appointment via automated phone system under "within 14 days of your next trip to overseas." You will be given the confirmation number, and the number will be verified by officers at the Federal Building. Bring two passport photos, your current passport, the marriage license/court order, a letter from UAL, your Crew Badge, new DMV license (or any Government-issued picture ID), and a check/credit card to the Federal Building. The necessary forms are available at the passport agency(you may download the form from the gov. web site if you wish). This is the same-day expedition process, and you will required to pay expedition fee(out-of-pocket). UAL only reimburses the regular passport fee as long as your passport is expiring within 60 days. If you wish the expedition fee reimbursement, your passport must be expiring within 30 days AND have your supervisor's approval by showing the past three months of LOF to prove that you had no reasonable time to renew your passport.

4.  UAL UG-100 Name Change - Bring your new passport to a supervisor; they will generate the UG100. Your name on the passport must match the name on IBS, so show your new passport every time you check in at the gate while you're on a trip until your new crew badge is available.

5.  FAA Airman Certificate - FAA recommends updating name and/or address changes. Contact your local FAA Office for hours and/or appointments (usually Mon, Wed, Fri). There is no fee for name and/or address change. Bring your original (if available) FAA-issued Airman card, updated DMV license, and marriage license/court order. A temporary paper certificate will be issued in exchange for your old card, which is valid for 120 days. Retain the paper certificate until the new card is mailed to you.
