LAXSW New or Renewal Passport Fees & Passport Expedite Fees

(NOTE: The following information is reproduced for convenience from local domicile policies & procedures. Direct additional questions to a supervisor via phone or email).


F/As must use FAINFO in Unimatic to update passport information; OR contact the LAXSW Coordinator to update information.

 

Procedures:

If F/A calls Supervisor or Coordinator about passport renewal procedures, F/A is directed to contact the Passport Office at (877) 487-2778 for an appointment, and for any questions about passport renewal, hours of specific passport office, etc. Additional information may be found on the Internet at www.travel.state.gov. If F/A needs expedited passport renewal, Coordinators can provide F/A with a letter which will assist the process (see LAXSW Coordinator first).

New or Renewal Passport Fees/Passport Expedite Fees:

  1. F/As are eligible for new or renewal passport fees.
  2. F/As may also be eligible for expedite fee reimbursement. Expedite fee reimbursement forms are found at the Resource Center (at the domicile). Submit this form to any Duty Supervisor for signature authorization.
  3. For expedite fee reimbursement, Supervisor will require that F/A provide a copy of the previous 3 months line of flying to establish that F/A was unable to use the normal process for passport renewal.
  4. One week must normally be allowed for Supervisor to complete this expedite fee review process. Once approved, Supervisor will forward this signed authorization form to F/A. 
  5. Once F/A has a new passport in possession and has updated the Unimatic FAINFO screen, F/A may file with the Service Center (WHQUN) for reimbursement of passport fee and, if applicable, expedite fee. 
  6. To file for all reimbursements, use the F/A Reimbursement Form (USW4166). This form is in a self-contained envelope format, and is available at the Resource Center (in the domicile). A copy of the new passport, originals of all related invoices and, if applicable, the signed expedite authorization form, must accompany all requests.
  7. Send this F/A Reimbursement Form/Envelope to the WHQUN Service Center via Company Mail. 

    Note: Receipts for passport renewal or for expedite fees must be no older than 60 days, or they will not be approved by the Service Center.

Jury Duty Information

Jury Duty - Section 4.N of your Contract:

 

A F/A who is called upon to perform jury duty shall receive flight pay credit for any flight time loss (daily vacation rate for reserves). The F/A shall be available for rescheduling on those days originally scheduled for duty which do not conflict with the jury duty assignment; however, after five (5) or more consecutive days of jury duty, or combination of flight duty and jury duty, the F/A will be guaranteed two (2) additional days off with pay. Any compensation or expenses provided by the court may be retained by the F/A.

New Unimatic Codes for Jury Duty


Beginning June 12, 2007, two new Unimatic codes, JDA and JDU, will replace the use of ABS and DNF when a F/A is removed from the schedule for jury duty. JDA will be used to reflect a removal from schedule when jury duty authority exists in the work history and JDU will be used when removed from schedule for jury duty without authority. These changes make the Unimatic line of flying and work history (FDWH) codes consistent. The new codes also allow for better tracking and trending of trip removals and staffing requirements for resource planning purposes by separating jury duty from other uses of ABS and DNF.

The following reposted from LAXSW Domicile Procedures - additional inquiry must be directed to a LAXSW supervisor or coordinator.

 

Obtain Jury Duty form from the LAXSW Coordinators Desk or a Supervisor. You must provide a copy of your Jury Duty summons.

 

  • Once you receive your summons, you may not take action which would increase your scheduled flight hours within the period of your jury duty assignment.
  • Your Jury Duty authorization is valid for 5 days. If your Jury Duty exceeds 5 days, you must contact the LAXSW Coordinator to extend your authorization.
     
  • You are subject to reassignment on those days originally scheduled for jury duty, which do not conflict with the jury duty assignment. Should you be “excused” from jury service during regular workday (Mon - Fri) and you have trip days remaining during the excused period, you must contact OPBSK for possible reassignment. (Jury service plus trip days not-to-exceed 5 days.)
     
  • You are required to notify the Service Center (FLT-LINE) prior to each scheduled ID/RSV day to keep them informed of your status.

Once you have been released:

 

  1. On your last day of service, request a record of jury duty service (usually a computer printout). The court will provide you with this verification of jury duty service upon request. Please turn in the original record to the LAXSW Coordinators desk and keep a copy for your files. Any compensation of expenses provided to you by the court may be retained. Verification of service may be faxed to the Coordinators attention at: FAX: (310) 342-8675
     
  2. Contact OPBSK immediately upon your release from jury duty.
