CAIC Voluntary Supplemental Plans (AFA-CWA Sponsored)

Annually, AFA-CWA and NGP coordinate with United management to provide an opportunity for NGP representatives to access our larger bases and our AFA Members for the purpose of reviewing and enrolling in various benefit plans, should Members find a plan that is beneficial to their individual needs. Enrollment is additionally available online or by telephone during this annual enrollment period.  Open enrollment for 2025 closed at the end of April and provided these additional benefits for our Members. Reports indicate we had a very successful open enrollment period again this year.

The plans available to our Members are underwritten by Continental American Insurance Company (CAIC), and claim forms are processed by CAIC.

While AFA and our broker, NGP (National Group Protection), have a collaborative relationship in providing active AFA Members an opportunity to enroll in these voluntary supplemental plans, AFA does not administer the plans or process any claims.

To clarify the claims process, please review the following:

Step 1

Once there, click on the yellow CAIC Benefits & Claim Forms tab.

Step 2
Access the claim form.
Complete the online claim form and follow directions to send the complete form directly to the CAIC address. 

Note:  Although there is an area on the form for a supervisor or AFA representative signature, we have worked with NGP to expedite this process and avoid any delay obtaining a signature.  AFA and NGP will coordinate any signature requirement.  As a result, you are no longer required to obtain a signature prior to submission of the claim form.
